prescription.
Up to a 30-day supply of the medications on
the formulary can be purchased
for a $4.00 co-pay regardless of insurance coverage.
In most cases, 30 or 60 tablets will make up a
30-day supply of these items depending on typical prescribing habits.
Some medications taken only “as needed” and the $4.00 copay may only
include one of these per prescription.
Examples of savings:
Furosemide 40mg 30 tablets=$4.00
taken once daily
Furosemide 40mg 60 tablets=$8.00 taken twice daily
Ranitidine 150mg 30 tablets=$4.00
taken once daily
Ranitidine 150mg 60 tablets=$4.00 take twice daily
Hydrocortisone 2.5% Cream 30gm=$4.00
Hydrocortisone 2.5% Cream 60gm=$8.00 (2x30gm)
